Baron-Cohen Disowns the 'Extreme Male Brain' Label as Cambridge Lands £26m Autism Gift

Professor Simon Baron-Cohen has publicly distanced himself from one of the most cited frameworks in autism research, telling The Guardian on 5 July 2026 that he regrets using the term 'extreme male brain' and no longer considers labels like 'male brain' or 'female brain' useful. The statement arrives alongside the announcement of a £26 million ($34.5 million) donation from US philanthropist Lisa Yang to Cambridge University — the largest single private gift for autism research in the institution's history — which will establish the K Lisa Yang Centre for Autism Research and a clinical autism centre within the future Cambridge children's hospital, both under Baron-Cohen's directorship.
The retraction carries real weight. Baron-Cohen has been the theory's principal architect and advocate for roughly three decades. Cambridge's own press office was citing his work as providing "strong evidence in support of the extreme male brain theory of autism" as recently as 2014, and a 2018 Cambridge study involving more than half a million participants was framed as the largest-ever investigation of psychological sex differences, explicitly tied to the EMB framework. As of this writing, Cambridge's public-facing research pages continue to promote those findings; no institutional statement revising or contextualising that body of work has been issued.
The Empathy Myth, Corrected
Baron-Cohen's specific concern is downstream harm from the terminology. He argues that 'extreme male brain' generates reductive press coverage — chiefly the claim that autistic people lack empathy — which he calls a myth. His distinction is clinically precise: autistic individuals tend to diverge on cognitive empathy (the capacity to infer another's mental states) but not on affective empathy (the capacity to feel concern or distress in response to others' emotions). Conflating the two produces a caricature that, beyond being inaccurate, carries social costs for autistic people navigating relationships, employment, and healthcare.
The framing matters to researchers too. The EMB theory has long attracted criticism from scholars of gender and neurodiversity who argued that mapping neurological profiles onto a male/female binary was both conceptually imprecise and potentially stigmatising for autistic women and non-binary individuals. Baron-Cohen's own data has complicated the neat male/female typology: a 2015 Cambridge study found that women with autism also scored in the 'extreme male' range on the Eyes Test of mindreading — a result that, depending on your prior, either confirmed or undermined the theory's underlying sex-difference logic.
New Money, New Research Agenda
The Yang donation reorients what Baron-Cohen's team will actually be studying. Among the findings cited to accompany the announcement are unpublished, non-peer-reviewed data from 141,672 individuals suggesting autistic women face a 71% higher risk of serious cardiovascular events — heart attack, stroke, and related cardiac outcomes — compared with non-autistic women. That figure has not yet cleared peer review and should be treated with corresponding caution; preliminary datasets of this scale can shift substantially through the review process. Still, the signal is striking enough that it appears to be shaping the new centre's clinical priorities.
The cardiovascular finding also reframes the policy stakes. Autistic women are chronically underdiagnosed — partly a legacy of diagnostic criteria developed predominantly on male samples — and if the cardiac risk differential survives scrutiny, the downstream implications for clinical screening protocols and NHS resource allocation are considerable.
The gap between Baron-Cohen's public statements and what Cambridge's institutional web presence still says about his work is worth noting. His faculty profile at Cambridge Neuroscience continues to reference the EMB theory approvingly, with no caveats. Universities move slowly on these things, and a single interview is not a formal retraction of a research programme. But the divergence between the researcher's current stated position and the institution's public record creates a quiet inconsistency that the new centre will, presumably, need to resolve — particularly as it begins recruiting participants and seeking further funding under Baron-Cohen's name.
What the Yang gift does is create the infrastructure for a more expansive, and potentially more corrective, research programme. The transition from a theory built on psychological sex-difference scoring to one centred on physical health disparities and clinical outcomes is a substantive methodological shift. Whether the new centre produces findings that formally supersede the EMB framework, or simply expand around it, will depend on what the data show — a standard Baron-Cohen himself has long invoked.
