Eight-year-old Lillian Smart of Ruston, Louisiana, died on Saturday, one day after celebrating her birthday. She had been infected with a rare organism called Naegleria fowleri, often described as a "brain-eating amoeba," after swimming in Lake Claiborne (The Guardian).

The Louisiana Department of Health confirmed on 19 August that a state resident had been infected and said the exposure most likely happened while swimming in Lake Claiborne in Claiborne Parish (LDH). Lillian had been hospitalized since 15 August. The Louisiana Illuminator had earlier identified her as a seven-year-old from Ruston hospitalized after being infected with the amoeba, which feeds on brain tissue (Louisiana Illuminator). The Smart family publicly asked for prayers after the diagnosis (WWL-TV).

The family announced Lillian's death in a public Facebook post on the "Love for Lillian" page. During her hospitalization, more than two dozen businesses in Ruston showed support and donated a portion of sales to the Smart family.

Naegleria fowleri causes a brain infection called primary amebic meningoencephalitis, or PAM. It can develop when water carrying the amoeba goes up a person's nose, allowing the organism to travel to the brain. The amoeba lives in warm freshwater, such as lakes, rivers, and ponds. It is not found in salt water or in swimming pools that are properly cleaned and treated with chlorine. According to the Louisiana Department of Health, exposure typically occurs when people go swimming or diving in warm freshwater (LDH). You cannot get infected by swallowing water that contains the amoeba, and the infection does not spread from person to person.

The pathogen is extremely rare. Only about 180 cases have been identified in the United States since 1937, according to the Centers for Disease Control and Prevention (CDC). In Louisiana specifically, only a handful of infections have been recorded since 2011. The most recent comparable case in the region occurred in 2023, when an Arkansas child died after contracting the amoeba from a country club splash pad in Little Rock.

Louisiana has also dealt with Naegleria fowleri in public water systems. The state's ACT 573 compliance reports were issued in response to three deaths caused by the amoeba that were associated with public drinking water (LDH, 2021). Those earlier cases were linked to tap water rather than swimming in freshwater. They prompted legislative and regulatory changes in how the state monitors and treats its water systems.

Experts say the climate crisis is causing Naegleria fowleri to appear in parts of the US where it is not typical, including northern and western states. Warmer water temperatures expand the areas and seasons in which the amoeba can survive and reproduce. That could increase the risk of exposure in regions that have not historically recorded infections. It also makes public health messaging harder, since awareness of the amoeba's risks has mostly been concentrated in southern states where warm freshwater conditions are most common.

PAM's fatality rate is high, and the time doctors have to act is short. Once the amoeba reaches the brain, the infection moves fast, and diagnosis often comes late. There is no standardized treatment that has been proven to work at scale, though experimental drug combinations have been tried in individual cases. For public health officials, the challenge is communicating a risk that is real but extremely low, without causing panic on one hand or making people too casual about freshwater swimming on the other.

The Lake Claiborne case follows a familiar pattern: a child exposed during warm-weather freshwater activity, a rapid decline, and a fatal outcome.

What may be changing is where this happens. If warming freshwater extends the amoeba's habitat northward, the systems built to track and respond to it, which are mostly in Gulf Coast states, may need to expand. Doctors in regions unfamiliar with PAM may also face a steeper learning curve in recognizing and treating it.

The broader picture is that one case, however devastating, does not mean an outbreak. But it does signal that the conditions allowing this organism to thrive are shifting. When a disease is both rare and almost always fatal, even a small change in where it can survive matters a great deal for the people responsible for public health planning.
