Telstra's triple zero outage: an old computer and a missed update

Telstra bosses have told a Senate inquiry that a national triple zero outage on Wednesday, 15 July 2026 could have been prevented. The outage lasted about five hours. The fix? Replace a 15-year-old server or install a software update that was never put on the machine (The Guardian, 17 July 2026).

CEO Vicki Brady and Gerard Tracey, Telstra's executive in charge of system reliability, appeared before a Senate committee on Friday, 17 July 2026. The committee is looking into triple zero service outages (The Guardian, 17 July 2026; ABC News, 17 July 2026). The inquiry was set up on 28 October 2025, with public submissions closing on 25 November 2025. The committee was given more time to finish its report on 23 March 2026, with a new deadline of 14 April 2026.

Brady told the inquiry the outage could have been avoided if Telstra had updated its software or kept proper records of changes made to the server. Tracey was more specific. "A newer piece of hardware operating in the same design that we intended to, the issue wouldn't have happened," he said. The server cost roughly $30,000 to replace (The Guardian, 17 July 2026).

A server is basically a powerful computer that handles important jobs — in this case, routing triple zero calls to the right emergency service. A software update is a fix that the maker releases to keep things running smoothly, like updating your phone when a new version comes out. Neither of these is complicated or rare.

Telstra's own numbers, shown at the hearing, say 58,835 triple zero calls got through during the outage and 604 calls failed (The Guardian, 17 July 2026). The outage hit services across the country and cut hundreds of people off from triple zero (The Guardian, 17 July 2026).

Two things went wrong behind the scenes. First, a software update was never put on the device that caused the outage, according to Telstra's own submission to the inquiry (SMH, 17 July 2026). Second, changes made to the server were never written down, so nobody had a record of what had been changed or why. Brady flagged these problems in a statement published on Telstra's blog on 9 July 2026 (Telstra Exchange, 9 July 2026).

This isn't the first time the committee has heard about triple zero failures. An Optus network update on 18 September caused an outage that stopped over 600 triple zero calls from getting through. Samsung Electronics Australia also gave evidence to the committee on 9 December 2025.

The broader context here is that the inquiry is looking at whether Australia's emergency call system is reliable across all the phone companies, not just one bad day at Telstra. Two separate outages, two different causes, the same result: Australians dialling triple zero and not getting through. The Optus failure was a security update gone wrong. The Telstra failure was an old server that a $30,000 replacement and a routine software update would have fixed. Neither is complicated. Both are the kind of boring maintenance work that doesn't make headlines but keeps important systems running.

For a phone company running emergency services, a 15-year-old server doing a critical job is a choice. Telstra's record-keeping around changes clearly failed. Brady basically said so herself by listing the two things that would have stopped the outage, both of them internal stuff-ups. Those 604 calls that didn't connect each represent someone in an emergency who couldn't get help. Whether that leads to new rules or penalties for Telstra is up to the committee's final report and the government's response.

The committee has now heard from both major phone companies involved in triple zero outages. Its findings will test whether phone companies can be trusted to manage their own emergency infrastructure, or whether the Senate should push for stricter rules on replacing old equipment and keeping proper records for a service Australians are told will always work when they pick up the phone.
