Eli Lilly's Q3 Report: Why the Guidance Matters More Than the Quarter

Eli Lilly reports its third-quarter 2026 results on October 29, 2026, with five consecutive quarters of earnings-per-share (EPS) beats behind it, meaning profit per share has come in above analyst forecasts each time. Yahoo Finance Analysts had expected third-quarter revenue of $22.61 billion, up about 28% from the year-earlier quarter. Investopedia
Full-year guidance is the number that moves a valuation model, the spreadsheet analysts use to estimate what a company is worth. In August, Lilly raised its 2026 revenue outlook to $85.0 billion to $87.0 billion. Lilly In April the range sat at $82.0 billion to $85.0 billion. The February initial range was $80 billion to $83 billion, with non-GAAP EPS guidance of $33.50 to $35.00. Non-GAAP figures are adjusted numbers that leave out certain items the standard accounting rules include. Both ends of the revenue range have moved higher at each update.
The second-quarter base is the relevant comparison. Revenue rose 48% to $23.0 billion, driven primarily by Mounjaro and Zepbound volume, and EPS rose 26% to $7.94. PR Newswire The $22.61 billion revenue estimate sits about $0.4 billion below that figure, which implies a sequential dip from the second quarter.
The gap is small enough that a modest beat or miss would move the year-over-year growth rate more than the sequential story. The sequential dip is worth noting, but it is unlikely to decide the quarter.
The product split is where expectations are most concentrated. S&P Global Market Intelligence projected Mounjaro sales of nearly $10 billion, up about 53% year over year, and Zepbound sales of $5 billion, up about 39%. S&P Global Those two projections total roughly $15 billion, or about two-thirds of the $22.6 billion revenue estimate.
Any shortfall in the incretin franchise, the drug family that includes Mounjaro and Zepbound, would therefore carry most of the weight in the headline number.
Earnings expectations have moved more than revenue expectations. Yahoo Finance reported that the third-quarter earnings estimate fell from $9.87 to $6.67 because of acquired in-process research and development (IPR&D) charges. Yahoo Finance IPR&D charges are one-time accounting entries recorded when a company buys early-stage drug assets that have not yet reached approval. They reduce reported profit in the period they are booked, so a quarter can look weaker on an EPS basis while revenue is growing.
Readers comparing per-share figures across quarters should check whether a number includes these charges and whether it is GAAP or non-GAAP. GAAP is the standard accounting framework, without adjustments.
The longer run-rate is strong. Third-quarter 2025 revenue rose 54% to $17.60 billion, with EPS of $6.21. PR Newswire Fourth-quarter 2025 revenue rose 43% to $19.3 billion, and first-quarter 2026 revenue rose 56% to $19.8 billion, primarily from volume growth.
The comparison base is rising each quarter, so percentage growth rates will narrow even if dollar growth holds. That is a math effect, not a sign of weakness.
In my view, the guidance language will matter more than the quarter itself. A company that has raised its revenue range at each of the three updates this year is signaling confidence in volume. The range is still $2 billion wide, though, and the third-quarter print will show whether the first-half pace is holding in the second half. If Lilly holds the $85 billion to $87 billion range, the implied fourth quarter would need to be large. If it narrows or lifts the range, the market will likely read that as the volume trend persisting.
Still, forecasts in this setting carry a real margin for error. Third-party product estimates are built on prescription data and assumptions about pricing and channel mix, and a single IPR&D charge can swing EPS by several dollars in one quarter.
