The gap between state and federal case counts tells you something important about how the U.S. tracks disease outbreaks. Cyclospora cayetanensis has roughly a two-week incubation period. The CDC builds in an assumed six-week lag between the moment symptoms appear and when a case enters national case-count systems—the time it takes for a person to seek care, get tested, have results verified, and be reported up the chain. States report their own tallies much faster, capturing probable and confirmed cases as clinicians identify them locally. Neither count is wrong. Both serve different purposes: states act fast, the federal system confirms chain-of-transmission before the numbers go national.
Michigan's outbreak has accelerated sharply since late June. The Michigan Department of Health and Human Services first confirmed the outbreak on July 1, when it reported more than 170 cases across seven counties—Monroe, Lenawee, Washtenaw, Wayne, Livingston, Shiawassee and Jackson. By July 6, the count had nearly doubled to around 700, according to ABC News. Within days, it had climbed to 1,251 by July 9, CIDRAP at the University of Minnesota reported. By July 13, the state's total stood at 2,640—a trajectory showing no clear signs of leveling off. A provider bulletin from July 7 showed 784 cases since June 22, concentrated in Monroe, Lenawee, Washtenaw and Wayne counties. Forty-four Michigan patients had been hospitalized as of July 9, according to state health department data updated daily on weekdays.
Ohio's outbreak is smaller but follows a similar timeline. The Ohio Department of Health reported 177 cases as of July 2, with 171 of those occurring in June, most clustered after June 20.
Neither state has identified a specific food source. The Michigan Department of Health and Human Services said as of July 5 it had not linked the outbreak to a particular produce grower or supplier. That gap in the traceback persists into mid-July. Michigan's health department has nonetheless issued precautionary guidance to restaurants and commercial kitchens in the southeast of the state, urging thorough washing or cooking of leafy greens, snow peas, certain herbs and raspberries—the produce categories most commonly implicated in past cyclospora outbreaks nationally, even without a confirmed vector in this case.
Dr. Natasha Bagdasarian, Michigan's chief medical executive, told the Associated Press that "there is clearly a linked outbreak happening right now." The statement stops short of naming a specific source but signals epidemiological confidence that the cases share a common origin rather than representing random background infections.
The broader context here involves constraints on public health infrastructure itself. Barbara Kowalcyk, an associate professor at George Washington University's Milken Institute of Public Health and director of its Institute for Food Safety and Nutrition Security, told the Guardian that funding cuts have slowed the response. She notes that understaffing means some patients are now being interviewed six to eight weeks after illness onset rather than sooner—a delay that weakens traceback interviews, since people's memory of what they ate fades over time. The backdrop is the Trump administration's March 2025 decision to cut $11.4 billion in grants to state and local health departments, funding that had underwritten pandemic-era surveillance systems now being repurposed for outbreaks like this one. Michigan's public health labs alone lost $5.5 million in federal funding, according to Bridge Michigan.
The tension between the two counting systems reflects a real trade-off in public health. Federal figures standardize comparison across states and confirm laboratory-verified transmission chains. State figures move faster because they capture probable and confirmed cases as clinicians report them locally, before CDC verification. The six-week reporting assumption baked into CDC methodology means the federal count of 843 will continue rising for weeks after the outbreak's actual peak has passed—a lag that narrows when local capacity is robust, but widens when it erodes. The CDC itself has said it expects the case count to increase.
For now, the outbreak's origin remains unresolved, and cases in both Michigan and Ohio continue to climb. Cyclosporiasis causes watery, often severe diarrhea, along with fatigue and cramping. Most patients recover without hospitalization.
