The World Health Organization has confirmed that the Ebola outbreak in the Democratic Republic of the Congo is now the largest in the country's history and the world's second-largest on record. As of 30 July 2026, the outbreak, caused by the Bundibugyo virus strain, has resulted in 3,605 confirmed cases and 1,587 deaths — a case fatality rate of 44 percent. BBC News; Al Jazeera

Authorities declared the outbreak on 15 May 2026 in Ituri Province, which borders South Sudan and Uganda. Two days later, on 17 May, WHO Director-General Tedros Adhanom Ghebreyesus elevated the crisis to a Public Health Emergency of International Concern (PHEIC). This formal declaration signals an event serious enough to require a coordinated international response. At that time, eight laboratory-confirmed cases, 246 suspected cases, and 80 suspected deaths had been reported in Ituri alone. The trajectory since mid-May has been steep. Between 3 July and mid-July, WHO documented an additional 664 confirmed cases and 376 confirmed deaths. In the most recent reporting week, the outbreak reached its highest weekly total: 567 cases and 296 deaths. WHO stated on X that the outbreak was "intensifying, with sustained transmission and continued increases in reported cases and deaths," warning that the weekly figures showed an "exceptional pace of transmission." The Guardian reported on 31 July that this is the fastest-growing outbreak in the history of the Ebola virus, with case numbers approaching the toll of the 2018–2020 outbreak in eastern DRC. That earlier epidemic, which also spread to Uganda, recorded 3,481 cases and 2,299 deaths, according to a 2024 WHO SAGE report — a case fatality rate of roughly 66 percent. The current outbreak has surpassed that earlier epidemic in confirmed case count, though the Bundibugyo strain's lower fatality rate means the death toll has not yet matched the 2018–2020 figures. The Bundibugyo strain is a critical variable in this outbreak. Unlike the Zaire ebolavirus, for which the rVSV-ZEBOV vaccine demonstrated efficacy during the 2018–2020 epidemic, there is currently no approved vaccine or therapeutic for Bundibugyo. Vaccine candidates are under development in the United Kingdom and Singapore. In the absence of a licensed countermeasure, the response relies entirely on surveillance, contact tracing, isolation, and supportive care. The geographic context compounds the difficulty. The outbreak is centered in Ituri and has spread to North and South Kivu provinces, regions long affected by armed conflict, mass displacement, and porous borders with Uganda and South Sudan. Ebola virus disease spreads through contact with bodily fluids and has killed approximately 15,000 people across Africa over the past 50 years. The 2014–2016 West African epidemic, centered in Guinea, Liberia, and Sierra Leone, remains the largest on record. A July 2015 WHO situation report from that earlier epidemic documented 26 confirmed cases in a single week, a pace the current outbreak has dwarfed more than twentyfold in its most recent reporting period. Ministry of Public Health data compiled as of 31 July recorded 1,556 deaths at a case fatality rate of 44.1 percent, broadly consistent with the WHO's 30 July figures. The convergence of sources on a 44 percent fatality rate is notable. Bundibugyo ebolavirus historically carries a lower fatality rate than Zaire ebolavirus, which typically exceeds 60 percent. The lower lethality, combined with the absence of vaccination, creates conditions in which transmission can persist longer in communities because a greater share of infected individuals survive and remain mobile.

The broader context here centers on the operational challenge. The PHEIC declaration was intended to trigger coordinated international resource mobilization, but WHO's characterization of the outbreak as "intensifying" nearly three months later indicates that containment has not been achieved. The combination of an unvaccinable strain, active conflict zones, cross-border population movement, and a transmission pace WHO describes as "exceptional" places this outbreak in a category that the 2018–2020 response did not fully face. The development of Bundibugyo vaccines in the UK and Singapore is a forward-looking effort, but the timeline for clinical validation, regulatory approval, and deployment under emergency use is measured in months at minimum. In the interim, the response's trajectory will be shaped by whether community engagement, contact tracing, and cross-border surveillance can keep pace with transmission that WHO has described as accelerating week over week.
