According to the Gaza Ministry of Health, cited by ANERA in November 2025, more than 71,000 people in Gaza live with diabetes. Roughly 25% of that population is insulin-dependent, based on WHO figures — meaning some 17,500 people require refrigerated, uninterrupted insulin supply to avoid diabetic ketoacidosis and death. Among them are over 1,000 children with type 1 diabetes, for whom there is no oral alternative; their condition is managed or it is fatal.
A Supply Chain Broken at Every Link
The blockade is the structural problem. Human Rights Watch documented in December 2023 that restrictions on goods entering Gaza were preventing sufficient medicine from entering the territory and disrupting the refrigeration infrastructure that insulin storage requires. Cold chain failure alone renders available stocks unusable; the problem is not only quantity but the conditions of delivery and storage.
By March 2026, the WHO reported that medical stocks in Gaza were critically low despite Israel's reopening of a key supply route — confirming that physical access to a crossing is a necessary but insufficient condition for restoring pharmaceutical supply. Trucks require fuel, warehouses require power, and the distribution network requires staff and security to function. The conflict has degraded all of these.
The WHO had already assessed on May 22, 2025 that Gaza's health system was at a breaking point as Israeli military operations intensified, and a UNRWA situation report from August 2025 noted that people with type 1 diabetes — including children — require daily insulin to stay alive and that supply availability had been severely impacted. The timeline is consistent: each successive assessment finds conditions worse than the last.
Children Bearing the Sharpest Burden
A WHO EMHJ study focused specifically on children with type 1 diabetes in Gaza found that the war had made adequate diabetic care functionally unavailable, compounding the condition with food insecurity and severely restricted dietary choices. For pediatric type 1 patients, glycemic management depends on a precise relationship between insulin dose, carbohydrate intake, and activity — a system that collapses when food supply becomes erratic and insulin stocks intermittent.
The humanitarian consequences of this collapse are not theoretical. AP News reported in late 2023 that approximately 14% of Gaza's population had diabetes with little to no treatment available. An earlier AP dispatch from 2022 described a diabetic patient in a refugee camp who had gone five days without an insulin injection — a clinical scenario that, if sustained, leads to coma and organ failure within days.
The broader humanitarian architecture has not held. A UN Central Emergency Response Fund report covering 2025 found that the humanitarian crisis across the occupied Palestinian territory deepened further that year, a characterization that encompasses but extends well beyond the diabetes emergency.
What makes this particular crisis tractable in principle but unresolved in practice is that insulin is not scarce globally — it is a generic, mass-produced medication. The shortage in Gaza is entirely logistical and political. Restoring cold-chain-capable supply corridors, guaranteeing safe passage for medical convoys, and securing fuel for health facility generators are all technically achievable steps. Whether the political conditions for those steps exist is a separate question, and one the verified facts alone cannot answer.
What the facts do establish is a durable pattern: successive international monitoring bodies — WHO, UNRWA, HRW — have documented the same deficiency across two and a half years, with each update recording deterioration rather than recovery. For Gaza's 17,500-plus insulin-dependent patients, that trajectory is not an abstraction. It is a daily calculus of doses rationed against uncertain resupply.
