TB's Unfinished Business: England's Gains Sit Against a Global Diagnosis Gap That Claimed Millions

England recorded genuine, measurable progress on tuberculosis over the past fifteen years — but the global picture that frames it is far grimmer, and recent data suggest the disease's reach has not contracted nearly enough.
TB remains the second leading single-agent infectious killer globally after COVID-19, with more than ten million people falling ill each year, according to the UK Health Security Agency. Yet in 2022, only 7.5 million of an estimated 10.6 million cases were actually diagnosed, leaving more than three million people with active disease outside health systems entirely, Reuters reported citing WHO data. That diagnostic gap is not a new failure — WHO identified roughly the same three-million shortfall as far back as 2012 — but its persistence through a decade of declared global TB strategies is a pointed measure of how little structural ground has been gained.
England's trajectory sits apart from the global trend, though not without its own complications. Public Health England, NHS England, and local authority partners drove a 38% reduction in new TB notifications between 2011 and 2017, from 8,280 cases to 5,102, according to UKHSA. By 2021, the figure had fallen further to 4,425 notifications — a rate of 7.8 per 100,000 population — per UKHSA. Case fatality proportions also declined over this period: the share of TB patients in England dying each year dropped from 7.1% in 2002 to 5.5% in 2014, based on UKHSA research data.
Those numbers reflect genuine public health infrastructure at work. Contact tracing, latent TB treatment, and targeted screening of high-incidence communities all contributed. But the system's limits were visible long before incidence peaked. Research published in 2011 found that port-of-entry screening for TB among immigrants arriving in the UK was missing the majority of cases — a structural problem that took years of policy adjustment to partially address.
The Global Arithmetic of Mortality
The mortality toll that sits behind incidence figures is worth holding in sharp focus. In 2014, TB killed an estimated 1.5 million people globally. Of those, approximately 400,000 deaths were officially classified as AIDS deaths because they occurred in HIV-positive individuals — a categorisation that, while clinically coherent, has historically complicated TB's visibility in cause-of-death reporting and affected how funding and political attention are allocated, as BBC News noted at the time.
The HIV-TB co-infection dynamic remains one of the disease's most intractable clinical and epidemiological features. HIV-positive individuals face a dramatically elevated risk of progressing from latent to active TB, and co-infected patients require coordinated antiretroviral and TB drug regimens that many high-burden health systems struggle to deliver reliably. The three-million annual diagnosis gap is not evenly distributed — it is concentrated precisely in the settings where co-infection rates and drug-resistant TB strains are highest.
What the Gaps Mean Now
England's falling caseload is real, but it is not insulated from global dynamics. High-burden TB countries account for a substantial proportion of the UK's foreign-born TB cases — a pattern that makes international diagnostic and treatment shortfalls directly relevant to domestic epidemiology. If the global pool of undiagnosed, untreated TB remains at three million or more annually, the pressure on importation pathways into low-incidence countries does not ease.
The WHO's End TB Strategy targets a 90% reduction in TB deaths and an 80% reduction in incidence by 2030, using 2015 as the baseline. The 2022 diagnostic data — 7.5 million found out of 10.6 million ill — suggest the trajectory needed to reach those targets is not being met. COVID-19 disrupted TB services severely between 2020 and 2022, compressing the diagnostic and treatment pipelines that had been slowly improving. Some of that disruption has since been recovered, but the WHO's own assessments characterise progress as incomplete.
For practitioners and policymakers working in TB control, the current moment requires distinguishing between what is tractable — England's continued investment in latent TB identification and targeted community screening — and what remains structurally unresolved: the global deficit in diagnostic access, the underfunding of novel TB drug development relative to disease burden, and the categorisation problems that have historically obscured TB mortality in HIV-endemic settings. England's curve is a proof of concept. Scaling that proof is an entirely different problem.
