Two people in Michigan have died from cyclosporiasis, the state health department announced on August 3, 2026, marking the first fatalities in what has become the largest recorded US outbreak of the parasitic intestinal illness.

Both deceased individuals had significant underlying health conditions that may have been affected by cyclosporiasis and dehydration, according to medical records reviewed by health authorities. The Michigan Department of Health and Human Services identified the two deaths as part of the ongoing outbreak on its infectious disease outbreak page.

The outbreak's trajectory has been steep. Michigan's case count stood at 11,234 as of August 3, an increase of 461 from the previous Friday's update of 10,773 reported on July 31. Hospitalizations held at 193 across both reporting dates. The scale is extraordinary: as recently as June 30, the state had recorded just over 170 cases with no identified cause. By July 4, that figure had risen to 572. By July 13, MDHHS was investigating 2,640 cases.

Wayne County reported the highest burden of any jurisdiction, with 1,379 infections. Adults aged 30 to 39 were the demographic hardest hit, accounting for 2,216 reported cases statewide.

Nationally, the picture is even broader. The US Centers for Disease Control and Prevention recorded 6,707 laboratory-confirmed cases of cyclosporiasis as of July 28, with more than 11,500 additional suspected cases awaiting confirmation or investigation. The CDC confirmed it is aware of the two Michigan deaths in individuals with underlying health conditions as part of the July 2026 outbreak. The outbreak spans at least nine states, according to the FDA investigation.

An FDA investigation linked the outbreak to iceberg lettuce sourced from Taylor Farms operations in central Mexico. As of an earlier MDHHS update dated July 5, no specific produce grower or supplier had yet been identified, and the largest case increases were concentrated in Southeast Michigan. The multistate cluster had been documented in a four-state grouping of Michigan, Ohio, West Virginia, and Kentucky, encompassing more than 400 cases in that cluster alone.

The US Department of Health and Human Services said it was aware of the Michigan deaths. The CDC separately confirmed its awareness of the fatalities. Michigan's state health department told Reuters in an email that there were no updates on the investigation into the deaths specifically.

The broader context here matters for public health officials tracking foodborne cyclosporiasis outbreaks. Cyclospora cayetanensis, the protozoan pathogen responsible, is endemic in many agricultural regions and has been repeatedly associated with fresh produce imported into the United States. The parasite's incubation period of roughly one week, combined with the delay between symptom onset, clinical evaluation, laboratory confirmation, and reporting, means that case counts in an active outbreak invariably lag behind actual infections. The gap between Michigan's 11,234 reported cases and the CDC's 6,707 confirmed national total reflects different case definitions, jurisdictional reporting timelines, and the backlog of suspected cases under investigation.

The identification of Taylor Farms' iceberg lettuce from central Mexico as the vehicle, while the outbreak continued to accelerate, points to the structural challenge in produce-linked outbreaks: contaminated product may have already moved entirely through the supply chain before an epidemiological link is established. By the time the FDA's investigation pinpointed a source, the implicated lots had likely been consumed or discarded.

For state and federal health authorities, the two deaths introduce a new dimension to the response. Cyclosporiasis is treatable with trimethoprim-sulfamethoxazole, but severe dehydration can complicate outcomes in immunocompromised patients or those with significant comorbidities. The concentration of cases among adults aged 30 to 39 is consistent with exposure patterns seen in restaurant-associated outbreaks, where working-age adults are disproportionately represented among patrons.
