The CDC has expanded the largest known cyclosporiasis outbreak in U.S. history to 15 states, adding Arkansas, Iowa, Missouri, Nebraska, New Hampshire, and North Carolina to the growing list of affected jurisdictions. Arkansas's health department independently confirmed to the Guardian that the state is among those newly included in the CDC's multi-state outbreak designation The Guardian.

As of July 28, the CDC had recorded 6,707 laboratory-confirmed cases of cyclosporiasis nationwide, with approximately 11,500 additional suspected cases awaiting confirmation. Cases have been reported in 45 states since May, with the heaviest concentration of suspected cases in Michigan and Ohio. Michigan alone has reported 11,234 outbreak-linked cases and has now recorded the first two U.S. deaths linked to the disease since the outbreak began The Guardian.

Symptom onset typically occurs about one week after infection and includes diarrhea, appetite loss, bloating, extreme fatigue, low-grade fever, stomach cramps, nausea, and vomiting. The fact that Taco Bell's lettuce supplier was separately linked to the multistate outbreak raises questions about whether a common upstream source, potentially at the growing or processing level in Mexico, connects what initially appear to be distinct outbreak clusters.

The geographic expansion pattern also warrants attention. Early iceberg lettuce-linked cases clustered in the Midwest and mid-Atlantic. The newly added states, stretching from New Hampshire to Arkansas, suggest either delayed detection, secondary distribution channels, or both. The concentration of suspected cases in Michigan and Ohio, combined with Michigan's disproportionately high case count of over 11,000, points to potential amplification through localized supply chains or population exposure patterns that investigators have not yet publicly detailed.

For public health officials, the key unresolved questions are whether all six outbreaks under investigation share a common contamination source, whether the case count will continue to climb as suspected cases are confirmed, and whether additional fatalities will emerge in states with high case burdens.
