The Titan Report: Why the Submersible's Hull Design Matters Three Years Later

Canada's Transportation Safety Board released its formal investigation report (M23A0169) on June 17, 2026, into the implosion of OceanGate's Titan submersible on June 18, 2023. All five people on board died during a descent to explore the wreck of the RMS Titanic in the North Atlantic.
This report arrives nearly three years after the vessel was lost, but it is not the only investigation examining what happened. The U.S. National Transportation Safety Board launched its own inquiry (NTSB case DCA23FM036), and the U.S. Coast Guard opened a formal Marine Board of Investigation — its most rigorous fact-finding tool short of a criminal case. The number of oversight bodies reflects the complexity of investigating a vessel that crossed international boundaries and the intense public and regulatory attention the disaster drew.
The Hull's Role in Every Investigation
Titan combined two different materials: titanium end-caps attached to a carbon fibre cylindrical pressure hull. From the moment investigators found debris on the seafloor, this design became the focus of intense scrutiny.
At NTSB hearings in September 2024, engineer Don Kramer testified that the carbon fibre hull contained wrinkles, porosity, and gaps that originated during manufacturing — not from operational wear. This distinction is crucial. Defects created during the layup and curing process exist from the first dive onward; they do not wait for an accident or operational mistake to appear. Carbon fibre composites are also vulnerable to delamination and microcrack growth when exposed to repeated cycles of pressure and depressurization. Each pressurization and depressurization accelerates crack growth in ways that standard non-destructive testing cannot reliably detect without specialized inspection methods.
Titan completed several dives before June 2023. Each cycle would have stressed any existing voids and wrinkles. The cumulative damage to the hull on its final descent cannot be known directly — the vessel imploded and was destroyed — but the material science governing carbon fibre under cyclic pressure is well understood.
Where Regulation Stands and What Comes Next
OceanGate operated Titan as an experimental submersible, a status that exempted it from the certification standards that commercial passenger submersibles must meet from bodies like DNV or Bureau Veritas. Before the fatal dive, critics — including some of OceanGate's former contractors — had raised concerns about the hull's lack of formal certification. The TSB report and the ongoing USCG Marine Board investigation both bear directly on whether that regulatory gap will be addressed through legislation.
The key question now is whether the three investigations produce enough agreement on the facts to shape unified recommendations for maritime flags, classification societies, and domestic lawmakers. The NTSB has a strong record of turning its safety findings into federal rules; the TSB works the same way in Canada. When the USCG Marine Board completes its work, Congress and the International Maritime Organization will be watching closely. Any lasting change to how governments and the industry regulate manned submersibles would need approval at the international level.
The loss of Titan was complete. There were no survivors and no intact pressure hull to examine. What remains are the debris collected from the seabed, the evidence gathered during the search, and the engineering analysis compiled across three jurisdictions over nearly three years. The TSB's report is a substantial contribution to the record — a structured analysis that will inform the regulatory conversation ahead, though the conversation itself is far from over.
