How the Netherlands Prepares for Deadly Heat: The System Behind the Warning

The RIVM — the Dutch National Institute for Public Health and the Environment — decides when to activate the Netherlands' National Heatwave Plan and alerts public health officials and local authorities to take action.
The plan was created in 2007 by the Dutch Ministry of Health and the RIVM. It arrived at an important moment: most European countries did not yet have formal heatwave protocols. That changed after the summer of 2003, when a prolonged heat event killed approximately 70,000 people across Europe, exposing the continent's lack of preparation. France responded first, launching its national heat plan in 2004. The Netherlands followed three years later, building a system suited to its own population and healthcare setup.
The key difference in the Dutch approach: rather than leaving activation decisions scattered among different cities or regions, the Netherlands gave RIVM—a single national institute with the expertise to track weather patterns and disease trends—the power to trigger alerts. When conditions meet set thresholds, RIVM sets off a chain reaction: healthcare providers get notified, municipalities activate their response, and networks supporting vulnerable people move into action. The design choice was deliberate: speed and consistency across the country mattered more than letting local areas make independent calls.
An evaluation by the European Environment Agency, published on 18 June 2026, confirmed that RIVM's role as the central trigger remains the backbone of the system.
But a centralised alert is only as effective as what happens next. The people most at risk during heatwaves are the elderly, those with heart or breathing problems, and isolated urban residents — groups that often cannot protect themselves without help. An alert only matters if it leads to real action: care workers checking on homebound people, cooling centres opening, clear public messaging reaching those who need it. Evaluations of the Dutch plan have repeatedly asked the hard question: does the chain from RIVM's alert all the way to those final interventions actually hold up under pressure?
The stakes of that question are rising. Dutch summers have warmed along with the rest of Europe, and heat events are now arriving more often, lasting longer, and reaching higher temperatures. A system designed around 2007 climate patterns is now operating in a materially hotter environment than its creators anticipated. Officials across Northern Europe are wrestling with this gap between what their plans were built for and what the climate now delivers — a challenge that demands attention sooner rather than later.
