Hunter Schafer has revealed she had bottom surgery last December.

“I just had bottom surgery in December, which was amazing, and I love it so much,” she told Elle magazine in an interview published on 19 October 2026. The conversation, titled ‘Hunter Schafer Has Never Felt More Like Herself’, is the original account of her decision, as reported by Variety.

Bottom surgery is a gender-affirming operation that alters a person’s genitals to align with their gender identity. Schafer discussed what led to her decision in the Elle interview, according to E! News.

She described a slow, carefully managed recovery in early 2026. Schafer rented a big apartment so friends and family could visit while she was required to lie flat. Her doctor limited her to 150 steps per day.

It is a rare, detailed account from an actor who has spoken openly about being transgender for almost a decade. Schafer was labelled male at birth and, at 17, spoke publicly while a junior at the UNC School of the Arts high school. In February 2025 she also said her new passport had been issued with a male gender marker following one of President Trump’s executive orders.

Schafer’s first acting role was Jules in HBO’s “Euphoria”, the teen drama that made her widely known. She went on to appear in “The Hunger Games: The Ballad of Songbirds and Snakes” and the horror film “Cuckoo”.

She will next be seen opposite Michelle Yeoh in “Blade Runner 2099”. The project is an Amazon miniseries, meaning a short series that tells one complete story across a limited run of episodes.

What makes this stand out is the directness. Schafer organised her own recovery space, counted her steps, and then told the story in her own words.
