The RIVM — the Dutch National Institute for Public Health and the Environment — is the agency responsible for activating the Netherlands' National Heatwave Plan. When summer temperatures spike, RIVM decides when to trigger alerts that tell hospitals, local governments, and support services to take action.

This plan was created in 2007 by the Dutch Ministry of Health and RIVM. The Netherlands was ahead of most European countries in building an official, organized response to extreme heat. The plan came after the summer of 2003, when a catastrophic heat wave killed roughly 70,000 people across Europe. France launched its own heat-response plan in 2004; the Dutch followed three years later with their own version, designed for how the Netherlands is organized and how its healthcare system works.

Why give this job to one national institute instead of spreading it across local towns and regions? The reasoning is straightforward: a single national agency can spot dangerous conditions and send alerts quickly and consistently. When weather forecasts meet RIVM's alert thresholds, the institute notifies hospitals, municipalities, and networks that support vulnerable people. This centralized system means the message goes out the same way every time, without delays or confusion.

The heat affects certain people most severely — the elderly, those with heart or lung disease, and isolated residents in cities where cooling is hard to find. These groups can't help themselves in extreme heat the way younger, healthier people might. That's why the alert itself is only the first step. What matters more is what happens next: care workers visiting elderly people at home, opening cooling centers where people can escape the heat, spreading the word through media and community networks.

A 2026 European Environment Agency case study on the Dutch system recognized RIVM's alert role as crucial to whether the entire plan works. Yet researchers have repeatedly asked a harder question: when RIVM sends an alert, do all those downstream actions — the home visits, the cooling centers, the public messaging — actually happen as they should?

The heat pattern across Europe has been changing. Dutch summers are growing hotter and heat waves are lasting longer. A plan designed around 2007 weather conditions now operates in a measurably different climate. Other countries in Northern Europe are grappling with the same gap — their old heat-response plans were built for conditions that no longer reliably occur. They're now working to adapt those plans to the new heat reality.
