Grey's Anatomy returned for Season 23 with a one-month leap, a luxury hospital wing and a new boss in charge.

The premiere is titled "Collide." It picks up one month after the Season 22 finale, the show's first time jump between seasons in four years, showrunner and executive producer Meg Marinis told Deadline. The episode streams on Hulu.

The story centres on the grand opening of a new VIP floor at Grey Sloan Memorial Hospital, the Seattle organisation where the series is set. The floor offers private suites, high-level security, hotel-quality bedding and access to private chefs, according to Variety. It was backed by Olympic Regent Health. Not everyone is convinced. Owner and attending surgeon Meredith Grey ends the hour with the warning: "This will implode."

The money behind the floor walks straight into management. James Wolk joins Season 23 in a recurring role, meaning he will appear in multiple episodes, as Seth Norton. Norton is a surgeon-turned-businessman linked to Olympic Regent Health. In the final reveal, he is named Grey Sloan's new Chief of Surgery. He takes the post left vacant by Teddy Altman.

Teddy and Owen Hunt are gone. Kim Raver and Kevin McKidd departed as Teddy and Owen in the Season 22 finale.

The change at the top sits alongside a run of personal turns. Simone discovers she is pregnant. Jo tells Link she has quit her job. Bailey names Winston residency director, putting him in charge of training junior doctors and making him intern Jules' direct boss, so Bailey can pursue a Masters degree. Amelia and Toni reconcile and get back together. Meredith is back at the hospital after last season set up wedding bells for her and Nick, played by Scott Speedman.

Attention now stretches beyond Seattle. ABC has given a straight-to-series order, meaning no pilot is shot first, to an untitled spinoff set in West Texas. A spinoff is a new series built from an existing show. The order is for midseason, with the series scheduled for the 2026-2027 season.

The new series is co-created by Marinis and Grey's Anatomy creator Shonda Rhimes. The pair are writing it together and serve as executive producers, alongside star Ellen Pompeo, Marinis told The Hollywood Reporter. It is set at a rural West Texas medical centre described as "the last chance for care before miles of nowhere." Texas is Marinis's home state. She said the show will explore medical deserts and the gap between people who can afford excellent healthcare and people who cannot. Grey's Anatomy's previous spinoffs were Private Practice and Station 19.

Marinis also addressed how long the flagship can continue. She told The Hollywood Reporter that no one has told her when the final season will be. The decision on when to end is intended to be collaborative with Rhimes.

For fans, this means two tracks to follow. Seattle gets a reset with new leadership to realise. Texas gets a fresh hospital with very different pressures.
